Most common reason is a wrong / not sufficient JVM setting. Make sure you have configured enough memory for tomcat (min 512MB, better 1GB or more for -Xmx) and enough PermSize. Default PermSize is 64MB, but this is not sufficient. Better use a MaxPermSize of 256 MB.
